CONTINGENCY SUBMISSION FORM
Congratulations on your tournament win. We are excited to be able to present you with your contingency winnings. Below is an online form you need to fill out in its entirety. Also, we require the appropriate tax forms for the US or Canada. Those can be located below. Please fill out and attach to your submission. All contingencies are processed at the end of the month submitted. Please allow 6-8 weeks from the time of processing to collect contingency.
NOTE: All contingency claims must be submitted within 30 days of the completion of the tournaments. Any submission outside of the 30 days will not be eligible for contingency payment.
